The special conditions supplement with fees and deposit form attached must be used with applications that result in special requests/conditions that are not listed on the Facility Use Permit.

  2. Applications for community centers, park use, athletic or other facility or equipment use and payment of fees and deposits/damage bonds will be accepted only at the Park and Recreation Office at 201 North "E" Street, Suite 301, San Bernardino CA 92401. (In person only - No credit cards can be used over the phone)

  3. Date of application should be the date submitted. Applications are accepted on a first-come, first-served basis, therefore the applicant must pay the required fees in order to reserve the facility.

The only exception to paying prior to locking in the reservation will be for organizations, which require approval by boards or committees or who must pay by purchase order.
